CDAHCC Holistic and Healthy Living Expo Vendor Application 

Completion of the following form, along with your payment, registers you to be a vendor at the Coeur d'Alene Holistic Chamber's Expo.

Contract Stipulations

1. Setup begins Saturday October 31 at 7:00 am. Please be set up by 9:45 am. Doors open at 10:00 am. Clean up must be complete by 8:00 pm Saturday night.
2. Vendors are required to staff booths until doors close at 6:00 pm.
3. Vendors are responsible for all state and city licensing requirement. Sales tax must be collected and reported for all retail sales. Sales forms will be provided.
4. Tables will be skirted and covered. You are responsible for covering any additional tables. Please plan accordingly.
5. No refunds for cancellations later than October 15, 2015.
6. Sponsors are not responsible for loss, breakage or theft of vendors' belongings.
