2013 APPCNC Annual Conference

May 15-17, 2013 | Greensboro, NC

Registration Deadline: Friday, May 3, 2013

No telephone or at-door registrations will be accepted. Pre-registration is required. 

Instructions:

Only one person can register at a time. 

Please complete the registration form in its entirety. CEUs will be issued through Greensboro AHEC. The last four digits of your Social Security Number are required to receive CEUs and will not be used for any other purpose.

Registration Options: 

Please select the registration option that includes all dates you want to attend. We have provided registration options for the most commonly selected combinations of events. If you do not see the combination that suits you best, please use the paper registration form available at http://www.appcnc.org/annual-conference. 

Workshop Selection: 

Conference workshop descriptions are provided in the brochure available at http://www.appcnc.org/annual-conference. 

Please select your 1st, 2nd, and 3rd choices for each workshop time. Workshop placements will be made on a first-come, first-served basis. Please note that the Thursday workshop entitled "How to be an Ally to LGBTQ Youth" is a two-part workshop and participants should select both parts. 

Continuing Education Units:

The Greensboro Area Health Education Center has awarded 0.45 CEU (4.5 contact hours) to participants attending the Pre-conference; 0.55 CEU (5.5 contact hours) to participants attending Day 1 and 0.3 CEU (3.0 contact hours) to participants attending Day 2.

Payment: 

Pre-payment is required. Online registration will give you the option to pay by credit card via PayPal or by sending a check to APPCNC. 

Confirmations:

You will receive an email confirmation. If you have not received confirmation of your registration by May 7, 2013, please contact Melinda DeJongh at 919-226-1880 x108 or mdejongh@appcnc.org to make sure you are registered. 

Refunds and Cancellations: 

Refunds, less a $50 processing fee, will be given out only if APPCNC receives cancellations in writing before May 6, 2013. After May 6, 2013, there will be no refunds for cancellations or no-shows.
