2018 PeaceJam SLAM-Miami Registration Form 

This registration form is for individuals and/or groups attending the 2018 PeaceJam SLAM on November 3,2018 at Miami Dade College's InterAmerican Campus. In addition to this online form, we will need a paper form for each participant that is signed by their parent and/or guardian. Please email or bring a physical copy of these forms for each participant.
