WRAAA 2017 Independent Living Awards 

Nomination Instructions

WRAAA has a tradition of recognizing individuals and organizations that demonstrate commitment and excellence in helping older adults and individuals with disabilities live independently through our annual Independent Living Awards. Currently, we are accepting nominations for the 2017 Independent Living Awards. Please be sure to take the time to nominate an exceptional individual or organization for one of the five prestigious award categories and include all required information.

AWARD CATEGORIES

  • Excellence in Service Provision recognizes an individual or agency (nonprofit or public) who has demonstrated extraordinary commitment and dedication to helping individuals achieve independence through services and supports.
  • Excellence in Business Practices recognizes a for-profit company who has demonstrated extraordinary commitment and dedication to helping individuals achieve independence through its products and services.
  • Excellence in Advocacy: The Benjamin Rose Institute’s Katz Advocacy Award recognizes an individual, organization or group for their exceptional efforts that contribute to the advancement of public policy in support of independent living for older adults and/or persons with disabilities at the state and/or national levels.
  • Excellence in Volunteerism: An individual, organization or group will be recognized for their outstanding volunteer contributions that support and/or promote independent living for older adults and/or persons with disabilities.
  • Excellence in Leadership recognizes an individual or organization whose exceptional leadership has helped foster growth and positive change in the independent living movement and/or the systems that support and promote independent living for older adults and/or persons with disabilities.

AWARD ELIGIBILITY CRITERIA

  • Nominees’ work must impact older adults and/or individuals with disabilities in Cuyahoga, Geauga, Lake, Lorain and/or Medina counties.
  • Nominees’ will be evaluated on the impact of their past and/or present accomplishments promoting independent living in the categories of service provision, business practice, advocacy, volunteering and leadership.
  • Agencies/organizations must be in good standing with WRAAA and its funding partners.
  • Posthumous and self-nominations will be considered.

SELECTION PROCESS

WRAAA Board of Trustees, through its Independent Living Award Ad Hoc Committee, will select award winners. The winners will be notified by mail and /or email.

INFORMATION FOR NOMINATOR

The nomination form must be complete for consideration. Selected winners will be honored at the WRAAA Annual Luncheon and Conference on April 20, 2017 at the Holiday Inn – Independence (6001 Rockside Rd.). Nominators will be asked to present the winner with the award.

NOMINATION DEADLINE

The completed nomination form must be received by WRAAA on or before February 24, 2017. The nomination must be either submitted online, faxed (216-621-9262), mailed (WRAAA, 925 Euclid Ave., Ste. 600, Cleveland, OH, 44115, ATTN: Stephanie Manning), or emailed (smanning@psa10a.org).
