ACA provisions include establishment of patient-centered medical homes, a model that facilitates partnerships among patients, primary care providers, and other members of the health care team. Learn how APRNs and RNs can assume significant leadership roles in the delivery of care through medical homes.
This symposium provides attendees an opportunity to understand the provisions and accreditation requirements for patient-centered medical homes, hear first hand from APRNs about innovative nurse-led homes, view quality performance data already gathered and analyzed, and acquire positioning and marketing strategies for nurse-led medical homes in the new health care environment that includes accountable care organizations.
Support for this project was provided by a grant from the Robert Wood Johnson Foundation
Symposium Goals:
This symposium will help bring the nurses’ perspective, as well as the perspectives of consumers, accreditors, payers and policymakers, to bear on the role of advanced practice registered nurses (APRNs) in solving the primary care needs identified during health reform. The meeting provides attendees with opportunities to understand the provisions and accreditation requirements for patient-centered medical homes, hear firsthand from advanced practice registered nurses about innovative nurse-led primary care homes, gain insight into key quality and performance data, and acquire positioning and marketing strategies for nurse-led medical homes in a new health care environment that includes accountable care organizations. In addition to increasing knowledge, we want to increase enthusiasm about the very positive role nurse practitioners can play as leaders of primary care health homes. We hope to encourage more nurses in advanced practice to pursue recognition as leaders of primary care health homes.
Intended Audience: Nurses at all levels of practice, management, and leadership.
Learning Objectives:
1. Identify best practices from described models of nurse-led primary care clinics or medical homes that lead to increasing access to quality care for consumers.
2. Describe one potentially successful pathway to ccreditation of a nurse-led medical home.
3. Identify approaches to implementation of recommendations from the IOM Future of Nursing Report.
