Chapter Monthly Report Form (2016-2017)

The NBS-AERho Chapter Monthly Report form is a brief summary of the chapter's monthly activities and contact information.  Please submit by the 10th of the following month.  The NBS-AERho "year" is divided into 9 reporting periods.  Those periods are identified along with the corresponding month in the form on the following page.  For recording purposes, please follow the instructions on how to enter your e-mail address along with the reporting period.  Those instructions, at the bottom of the next page, must be followed in order for the report to be filed in correct order in the online database.
