2017 WVACDA Junior High-Middle School STUDENT PARTICIPATION REGISTRATION

TEACHERS/DIRECTORS - you must register yourself separately  under the conference heading on WVACDA.org

Due date for student registration and payment of fees is December 15, 2016! (If mailing a check for fees, it must be postmarked no later than December 15, 2016!) Pay online or by mail. See form for mailing address for payment by check.

You must also mail the NOTARIZED Medical Release Form by December 15, 2016. The form can be found at wvacda.org and/or from your honor choir chair.

Register your student singers for the 2017 WVACDA Junior High/Middle School Honor Choir here.
Be sure to include: student's name as it should appear on the program AND the student's voice part.

Note: be sure to complete the shirt section as  well - singers' shirts are included in their participants' fee but must be ordered with this form!
