2023 Scholarship Program Application

March 31- April 2- Annual Education Weekend – Must be present at Award Disbursement: April 1, 2023  (Grand Harbor Resort, Dubuque, Iowa)

Eligibility Requirements

 Must be a member of Hemophilia of Iowa, Inc. (HOI) If you havent registered in 2023 please do.  Your membership must be submitted on or before March 15th, 2023 when scholarship applications are due. If membership is not submitted on or before March 15th, 2023 your scholarship will not be considered. Do not send your HOI membership application to the scholarship committee, please go online (www.hemophiliaofiowa.org) to access the application.  NO EXCEPTIONS.

  • Member must reside in the HOI service area (State of Iowa or if outside the State of Iowa, must seek care at the UIHC Treatment Center) unless prior arrangements are made.    
  • Must have a diagnosed bleeding disorder or be an immediate family member (sibling, child, or caregiver) of someone that has a diagnosed bleeding disorder (diagnosed bleeding disorder is defined as a person who is being followed with a treatment plan).
  • Verification of diagnosis may be required from your physician upon request (you will be notified if this is required).
  • High school graduate before or in 2023.
  • Be enrolled in a degree seeking program at an accredited college, university, or trade school in the United States during the 2023-2024 academic year for a minimum of 12 credit hours or considered full time.
  • You must be present at the Scholarship Award Ceremony to be held during the Annual Education Weekend (unless agreed upon emergency). March 31- April 2 – Award Disbursement: April 1, 2023  (Grand Harbor Resort, Dubuque, Iowa) 
  • ·Active Participation in Hemophilia of Iowa will play a role in determining scholarships.
General Scholarship:

($500 for non-affected person and $1,000 for affected person)

 Scholarship Requirement: 

  • Complete the online scholarship application (click the link below)
  • Involvement within the HOI bleeding disorder community. (i.e., Attending events, participating in the walk, advocacy for the bleeding disorder community, volunteering) 
  • A typed 500-word (minimum) essay that includes each of the following:
    • Explain your short-range goals and long-range career plans.
    • Personal background in relation to the bleeding disorder community.
    • Contribution to the bleeding disorders community. 

 

Memorial Scholarships:

 There are three Memorial Scholarships Available. The Dude Cremer, John Heisner and David Brustkern  Memorial Scholarships.

($1,000 for non-affected person and $1,500 for affected person) 

 Application Requirements

You only have to submit the online scholarship application once (please do not resubmit). To apply for these Scholarships direct contributions must have been made to Hemophilia of Iowa. (donations of time/gifts/talents) To apply for these in addition to the General Scholarship please submit the following:

  • The General Scholarship Essay
  • A letter of recommendation from a non-family member.
  • A letter of recommendation from one of the following: a Hemophilia of Iowa board member, member of Executive Committee, a Chairperson, or someone from the HTC verifying contributions made to HOI and/or support.
  • A typed paragraph of how HOI has impacted your life. 
Submission Deadline

Complete online application found on the HOI website before or on March 15th, 2023 to be considered

**If applying for the Memorial Scholarships supporting documents must be turned in by March 15, 2023 **

Applications will be selected based upon the following criteria:

  • involvement  Hemophilia of Iowa and in the bleeding disorder community
  • Personal/leadership/academic qualities 
  • Completion of application and associated materials

 


All selection decisions are final and are not subject to appeal.  Applications and supporting documents become the sole property of Hemophilia of Iowa, Inc. and cannot be returned.  Your submission of application and supporting documents gives permission for Hemophilia of Iowa to utilize quotes, in part or whole as seen fit for future Hemophilia of Iowa usage.

It is your responsibility to inform the board president Amber Smith of any changes to your application. This includes, but not limited to, change of school and full- or part-time status. All documents will need to be emailed to Amber Smith at hoi_president@hemophiliaofiowa.org.
