Project Advisory Committee Meeting

You're invited to join SCC's Project Advisory Committee (PAC) Meeting on December 8, 2017

We are pleased to present this quarter's topics and keynote speakers:  

HIGHLIGHTING SCC'S COMMUNITY HEALTH ACTIVATION PROGRAM (CHAP)

Amanda Chirco, M.Ed, Project Manager, Community Health Initiatives, Suffolk Care Collaborative, will give an overview of the SCC's Community Health Activation Program (CHAP), a CBO-led in-reach and outreach program aimed to identify, engage, educate and integrate the uninsured and low/non utilizing Medicaid populations into community based primary and preventive care services.

APPROACHES AND STRATEGIES OPERATIONALIZED BY COMMUNITY BASED ORGANIZATION PARTNERS

The 3 founding CBO partners of SCC's CHAP will each provide an overview of how CHAP has been implemented within their organizations. They will share their approaches to engaging, educating and integrating the target Medicaid and uninsured population into community based care, highlighting successes, challenges and future plans of the project that focus on wellness coaching to increase engagement, knowledge and skills in managing their health.

Tara Fredericks, Director of Special Projects and Janet Farfan, Community Outreach/Health Coach from  Association for Mental Health and Wellness.

The Association for Mental Health and Wellness (MHAW) was formed in 2014 with the merger of three successful Suffolk County organizations: Mental Health Association in Suffolk County, Clubhouse of Suffolk, and Suffolk County United Veterans.  By coming together as one organization, MHAW can now offer the region comprehensive services and advocacy that focuses on mental health as well as overall health and wellness.  MHAW participates in multiple DSRIP projects including Integrated Health (3ai) and CHAP (2di).  

Adrian Fassett, CEO/President, Anne Stewart, Director of Programs and Halim Kaygisiz, Director of Health Outreach Services from Economic Opportunity Council of Suffolk.

The Economic Opportunity Council of Suffolk is a multi-service agency with locations throughout Suffolk County.  For the past 50 years, EOC has assisted individuals and families with reaching a level of self-sufficiency through its various programs and services.  From providing Care Coordination Services to helping Veterans and their families, EOC’s mission is to connect people to needed services and resources in the community.  EOC has partnered with the SCC since the start of DSRIP and assisted with the planning and development of the CHAP program.  

Roberta Leiner, Chief of Patient and Community Engagement and Trevor Cross, Community Liaison from HRHCare.

HRHCare Community Health is a nonprofit, New York State licensed, federally qualified health center. Founded more than 40 years ago to provide care to the medically underserved population of Peekskill, New York and is now one of the nation’s largest community health center networks.  HRHCare is proud to serve as a Coalition Partner of the Suffolk Care Collaborative including an active role in the DSRIP Project 2di Community Health Activation Program (CHAP).

LEAN PROJECT UPDATES

Alan Cooper,  Ph.D, MBA, Founder and President of the Tudor Advisory Group, will provide an overview of Lean Process Improvement Methodology and its application to DSRIP project implementation.  Dr. Cooper has been working with three SCC partners over the last several months to train key team members and scope Lean projects at each organization.  He will give an update on the progress of the various projects.  

MAX-NY AND THE SCC:  TOGETHER BUILDING PATHWAYS TO BETTER HEALTH

Susan Jayson, LCSW, Director of Behavioral Health Integration, Suffolk Care Collaborative, will give an overview of the MAX-NY Program and its implementation in Suffolk County.

The Project Advisory Committee’s (PAC) mission is to promote collaboration and support the goals, strategies, and initiatives among the SCC partners with respect to improving the health status of Medicaid and uninsured communities in Suffolk County.
