Improving Health Outcomes:

Meeting the Basic Needs of Populations

Combating hunger and poverty is an essential component to tackle the determinants of health.   While valuable public benefit and services exist to support people in need they are hard to access and remain under-utilized.  In order to march towards true value-based care it will be critical to understand how to integrate health and human service supports that control costs and improve health, especially for older dual eligibles. Cracking the code to efficiently and effectively connect the right clients, to the right benefits and services at the right time is a necessary tool to improve population health.

This forum will provide an overview of new research produced by Benefits Data Trust (BDT) in partnership with academic and state institutions, which proves that access to Supplemental Nutrition Assistance Program (SNAP or Food Stamps)that  improves health and reduces health care costs for older dual eligibles.  The forum will also explore practical strategies and actions that payors and providers can take to ensure that patients are able to meet their basic needs  in order to improve their health.  

The Forum presenter, Ginger Zielinskie, MBA is CEO & President of Benefits Data Trust, where she leads a diverse and dedicated team committed to transforming how people in need access public benefits and services. Committed to cross-sector collaboration, Ginger works with states, cities, the private sector and community-based organizations to understand the true outcomes that can be achieved when people are able to meet their basic needs.
