Collaborative Meeting

We look forward to working together to improve care coordination for your community! Please join us for the San Juan Care Transitions Collaborative meeting on Tuesday, June 16, 2015 from 2:00p.m. - 3:30p.m.

Intended Audience:

• Hospital leadership (case/care management staff, quality teams)
• SNF Leadership (medical directors, administrators, directors of nursing)
• Primary Care Physician offices, Home health agencies, dialysis centers
• Community partners

Meeting Goal and Objectives:

To build a community coalition engaged in improving the coordination of care for San Juan residents across settings. At this meeting, the coalition will:

1. Acknowledge existing charter with members that aligns collaborative efforts to improve care coordination
2. List interventions to address leading barriers outlined from participant survey results affecting coordination of care in the San Juan community
3. Identify concerns relating to medication safety and adverse drug event prevention
