The Population Health sessions will include a discussion of how one local company has culled a 98% participation rate of employees utilizing biometric screenings and correlating that with health assessment data; a session that explores the integration of social determinants of health into a regional CCO and HIE; and a session that looks at the development and utilization status of financial and clinical data by an Oregon based provider/payer run enterprise.

Attendees will have the option at lunch to participate in table topics led by a facilitator. The goal is to foster a discussion of key Healthcare IT topics to enable attendees to share experiences and identify tactics they can incorporate in their job. Topics will include: patient engagement, care coordination, telehealth, data security, data center management, data analytics, interoperability, information exchange, mobile device management and optimizing EMRs.
