REGISTRATION INFORMATION

Do not contact Minnowbrook directly for room reservations. Overnight accommodations are limited to 56 guests which includes shared rooms. Only a limited number of single rooms are available. Those staying overnight must register with the CEHD no later than Friday August 23rd, and indicate individual(s) who you prefer to be paired with to share room. The CEHD must provide a final “Rooming List” to Minnowbrook by this date. Members requiring single rooms or have special needs affecting lodging assignments should specify on registration site or contact Wade Silkworth directly by phone (585-753-5470) or email; WadeSilkworth@MonroeCounty.gov

CEHD Meeting Package Rate is $144 per night, and includes; Overnight lodging, dinner day of arrival, breakfast & lunch, all breaks and evening snacks. 2019 State per diem rate for Hamilton County is $94/night for lodging and Minnowbrook charges $50 for meals.

Payments for accommodations with meal package shall be made directly to Minnowbrook Conference Center. Tax exempt form ST-219 must be provided with payment to waive tax on room. Cash, check, credit card or vouchers are accepted.

Commuters may purchase meals by payment to the CEHD at time of registration. Meals are available at $13 for breakfast, $14 for lunch and dinner for $23 per person.

Meeting registration fee of $10 shall be collected from all attendees, payable to the NYS CEHD.

NYS CEHD Annual Dues of $20 per member (maximum fee of $40 per LHU / DO for multiple CEHD members) are due at Spring meeting, payable to the NYS CEHD by cash, check or voucher.

Payments for commuter meals, registration fee and dues may be made at check-in prior to Technical Session, or mailed in advance to the attention of CEHD Treasurer Aaron Lazzara, c/o Madison County Health Department, PO Box 605, Wampsville NY 13163.
