“Managing the Patient with Heart Failure: The Role of the Case Manager as a Team Member” 1.0 CE

Nancy Skinner, RN-BC, CCM, Princial Consultant, Riverside HealthCare Consulting, National Case Management Educator

Nancy Skinner, RN-BC, CCM, for the past 25 years served as a National Case Management Educator.  In her current role as Principal Consultant for Riverside HealthCare Consulting, she develops programs that advance excellence in care coordination and other transitional care strategies.  In 2002, she was named the Case Management Society of America (CMSA) “National Case Manager of the Year;” and in 2008 she received “CMSA’s Lifetime Achievement Award.” She has been active in CMSA at both the local and national level including serving as a National Officer of CMSA from 1995 to 2000 and 2004 to 2007. She serves as an Advisory Task Force Member of the National Transition of Care Coalition, a Director on the Board of the Hospital Quality Foundation, and acts as primary faculty for the University of Southern Indiana Case Management Certificate Course.

Paul P. Dobesh, Pharm.D., FCCP, BCPS (AQ Cardiology), Associate Professor of Pharmacy Practice, College of Pharmacy,  University of Nebraska Medical Center

Paul P. Dobesh, Pharm.D., FCCP, BCPS, is Associate Professor of Pharmacy Practice at the University of Nebraska Medical Center in Omaha, Nebraska. Dr. Dobesh earned both his Bachelor of Science in Pharmacy and Doctor of Pharmacy Degrees from South Dakota State University. He completed a specialty residency in internal medicine at the University of Texas at Austin at Brackenridge Hospital.

Dr. Dobesh’s current responsibilities at the University of Nebraska Medical Center (UNMC) include clinical practice in both internal medicine and cardiology services. He is responsible for teaching pharmacy and medical students, as well as residents. He lectures about ischemic heart disease, antithrombotic therapy, and other cardiology and critical care topics. Dr. Dobesh recently received the UNMC College of Pharmacy Educator of the Year Award.

Dr. Dobesh has conducted research on antiplatelet and antithrombotic therapy, focusing on the real-world use of these therapies and health-care economics. He has also published book chapters and several manuscripts in this field.

"Empowering Patients With Diabetes to Successfully Acheive THEIR Goals" 1.0 CE

Dr. Dave McCulloch, M.D. F.R.C.P.  
Medical Director of Clinical Improvement and Prevention, Group Health Cooperative

"Living with diabetes puts a tremendous burden on people. Their lives become filled with complexity and misunderstanding.  If they test their blood glucose at all, the results often don’t make sense. If they go see clinical providers to get help they often receive conflicting advice and leave feeling guilty that they are a “bad diabetic” and that this is all their fault.  If they go on the internet they get bombarded with misinformation and false promises.  Dr. McCulloch will show you a patient-centered approach that helps patients identify the most important priorities for their own lives and helps then understand how to manage their diabetes in ways that will lead to better health."

Dr. McCulloch practices with the Group Health Cooperative of Puget Sound. He serves as Member of Medical Advisory Board at WorldDoc, Inc. He is recipient of numerous awards and honors in the field; “Seattle Magazine” voted him as one of Seattle's best doctors. Since 1995, he has consistently been included in “The Best Doctors in America: Pacific Region.” He is board-certified in internal medicine. Dr. McCulloch is graduate and fellow of the United Kingdom's Royal College of Physicians.

"Medicare Spending Per Beneficiary" 1.0 CE

"The Community Integration Model" 1.0 CE

"Medicare Spending Per Beneficiary and The Community Integration Model offers a clear view of what current and pending Medicare mandates mean for acute- and post-acute care providers in the post-Affordable Care Act (ACA) landscape.  Medicare Spending Per Beneficiary or MSPB is the newest hospital penalty that was recently introduced by CMS.  This penalty focuses on the case management department and the case managers in that department and could change the way we look at data, best practices and patient choice.  The Community Integration Model (CIM) is a disruptive innovation to the current Continuum of Care. The CIM is an alternative discharge planning model that enhances patient choice while reduces Medicare Spending Per Beneficiary Measures.  It is a different way of thinking and processing when thinking about patient’s right to benefits vs a patient’s right of choice.  The CIM is currently being used in over 50 hospitals across the nation and this presentation will discuss aspects of the model as well as some surprising outcomes.  You will come away with a different way of thinking about what true patient choice looks like."

"COPD: The State of the Disease in 2016 and Beyond" 1.0 CE

Greg Ledgerwood M.D. AFP, ACAAI, AE-C

"I will be discussing the state of COPD as of 2016 including a review of the it’s pathophysiology, current and past methods of making a diagnosis, and recognizing factors, both familial and environmental, that influence the prognosis of this disease. I will also discuss the importance of recognizing exacerbations and what they mean for future COPD prognosis. Lastly, I will discuss both old and new treatments for this disease, and how a “team approach” can improve the quality of life for the patient and their family."

"Poster Presentations" Attendees will have the opportunity to earn 1 to 2 CEs by participation in the poster presentations at the exhibitor fair from 7:45am to 9:45 am and during morning and lunch breaks. It take approximately 1 hour to earn each poster presentation CE.
