Biographical Sketch: Dr MacDonald received his initial Physical Therapy (PT) education in Australia, and continued his education with the completion of his Doctoral Degree in PT from Regis University. He has obtained and re-certified status as a Board Certified Specialist in Orthopedics and Geriatrics within the PT field. He has also obtained Fellowship status in Orthopedic Manual Physical Therapy (OMPT). A recipient of multiple national research awards, he is currently the Director of the Regis Fellowship in OMPT and serves the PT profession as a chief delegate and an appointed panel expert in the field of PT including hip interventions.  As a speaker he has spoken at national and international conferences, on topics including biomechanics of the lower extremity, manual therapy and exercise interventions for the shoulder, hip labral impairments and fellowship training outcomes in PT. He currently resides in Colorado and provides care in an active outpatient PT clinic serving a wide spectrum of clientele.

Course Outline – An evidence based lecture and lab format two day course with an emphasis to current best evidence interventions for physical therapy management of the shoulder and hip complexes. Primary impairments of interest will be hip and shoulder DJD, shoulder and hip labral impairments, post-operative and conservative management and biomechanical considerations for best management approaches in both regions. A regional approach with inclusion of specific exercise and objective assessment tools will be undertaken. Emphasis will be placed on manual therapy interventions, including thrust and non-thrust approaches. Due to the inclusion of progressive manual therapy interventions the course is limited to licensed physical therapists.

Signing of a waiver for participation is required.

Course Objectives:

1 – Identify optimal screening approaches within PT care for shoulder and hip patients
2 – Develop diagnostic skill sets for shoulder and hip patients for extra vs. intra-articular presentations
3 – Develop manual therapy interventions in both regions, inclusive of select thrust and non-thrust interventions
4 – Expand knowledge of applicable research for both regions within the PT field and beyond
5 – Identify appropriate outcomes tracking tools and methods to assess both intra and inter-session progressions or lack thereof in response to PT management
6 – Expand upon specific exercise interventions for both the hip and shoulder regions
7 – Be exposed to novel perspectives on how to view the hip and shoulder complex from a biomechanical perspective

Structure: Two-Day Lab and Lecture Format – 16 hours, 16 CEU by hour.

 Schedule: Day One

Course Introduction - SHIP
Hip/pelvic, shoulder/scapula relevant anatomical review
    * Biomechanical considerations as needed
Screening for non-musculoskeletal impairments both regions
                  * Practical screening for lumbar, SIJ, knee and neurological presentation hip pain
* Practical Screening CS and TS for the shoulder
Clinical assessment for hip impairment differential diagnosis and shoulder impairment differential diagnosis
               * Establishing a functional baseline for re-assessment in both regions
               * Specific tests and measures for the identification of hip OA
               * Specific tests and measures for the identification of hip labral pathology
               * Specific tests and measures for the identification of intra vs. extra-articular shoulder pathology
Break
Lab session for practical instruction of manual assessment hip and shoulder impairments
                * Practical session for hip OA assessment
                * Practical session for hip labral assessment
                * Practical session for shoulder RTC, biceps, labral, OA vs. referred and neural assessment
Discussion diagnostic assessment for Hip pathology/impairments and shoulder impairments – radiographs, MRI
Lumbar and knee interventions to support hip conservative management

Cervical and thoracic interventions to support shoulder conservative management

Day Two

Morning review of techniques from day one
                * Practical review hip assessment techniques and shoulder assessment techniques
Evidence review for PT management of hip pathology, primary OA
                * Practical manual and exercise interventions for hip OA management
Evidence review for PT management of shoulder impingement and intra-extra articular impairments
                * Practical manual and exercise interventions for shoulder impairments
Break

Evidence review for PT management of hip labral impairments
                *Practical manual and exercise interventions for hip labral impairments
Special topics discussion PT management post RTC repair and total shoulder arthroplasty
