Interest Intake Form

Are you interested in having your child participate in the 2023 Middleton Tri4Schools/Arc-Dane County adaptive triathlon?  Fill out this form and get connected with The Arc-Dane County event chair Kim Caul.  Please note that this is not a registration form, but a way to indicate your interest and to give Kim some background on your child's needs.  Kim will reach out directly to follow up with applicants.  

   Deadline for submission to be considered for the 2023 event is March 27th.
