About The Program

This two-day seminar led by Mark Galley from ThinkReliability will teach you to use Cause Mapping to identify all of the interrelated causes that led to an incident and come up with solutions and prevention strategies.

ThinkReliability specializes in applying Root Cause Analysis to solve all types of problems. We investigate errors, defects, failures, losses, outages and incidents in a wide variety of industries. Our Cause Mapping analysis method of root causes, captures the complete investigation with the best solutions all in an easy to understand format. ThinkReliability provides investigation services and root cause analysis training to clients around the world and is considered the trusted authority on the subject.

In the Cause Mapping method, the word root, in root cause analysis refers to the causes that are beneath the surface. Most organizations mistakenly use the term "root cause" to identify the one, main cause. Focusing on a single cause can limit the solutions set resulting in better solutions being missed. A Cause Map provides a simple visual explanation of all the causes that were required to produce the incident. The root is the system of causes that reveals all of the different options for solutions.

The Cause Mapping method is an extremely effective systems-thinking approach to root cause analysis that improves the way people collect and organize information. It minimizes terminology and buzzwords by focusing on basic principles. Clear communication is required for solving problems effectively.

We teach
individuals how to document information with pen and paper, using dry-erase boards, on chart paper or electronically in Microsoft Excel. The Cause Mapping method is used in a variety of industries by front line employees and management to prevent problems and improve results.
