| COST: | $80.00 per person, includes golf, cart, hot dog lunch, dinner and prizes |
| DEADLINE: | Registration and payment must be received by Gary Reilly, 819 Moseley Road, Fairport, NY 14450, no later than Friday, August 30, 2018. |

- 1st Place*: Men's; Ladies; Mixed Team

- Nearest to the Pin
- In addition, a Paul Harris drawing for clubs w/ at least a paid foursome.
Please note: *To be eligible to win first place, a team must have at least three (3) registered players who will finish the event.
NOTE: Each Club is requested to donate a raffle item valued at $25.00. Please bring the raffle item with you on the day of the tournament. No time to shop? You may send a check, in the amount of $25.00, to Gary Reilly no later than August 27, 2018 and he will do the shopping for your Club.
Please remember the net proceeds will benefit DG Mike’s initiative on Opioid Overdose Epidemic and Human Trafficking.
